The Complete Boarding Intake Checklist

1. Vaccination Records. Rabies, bordetella, DHPP at minimum. Influenza and leptospirosis if you require them. Verify expiration dates, not just the name of the vaccine.

2. Feeding Instructions. Brand, type (wet/dry/raw), amount per feeding, feeding frequency, any food sensitivities or allergies. If the owner is bringing food, confirm it is labeled with the dog's name.

3. Medication Schedule. Drug name, dose, frequency, administration method, time of day, and any special storage requirements. Have the owner demonstrate administration for unusual medications.

4. Emergency Contact. Must be reachable during the entire stay. Not just a cell phone — a backup contact if the primary is unreachable (on a cruise, in surgery, etc.).

5. Veterinarian Contact. Practice name, phone number, attending vet name if known. Critical for medical emergencies when the owner is unreachable.

6. Behavioral Notes. Dog aggression, fear triggers, handling preferences, past incidents, separation anxiety, escape history. These notes go on the kennel run card and must be visible to every staff member.

7. Exercise Instructions. How many walks, how long, on-leash only vs. off-leash play group eligibility, any exercise restrictions.

8. Signed Boarding Agreement. Vaccination acknowledgment, emergency medical authorization with dollar limit, cancellation policy, liability limitations.

Frequently Asked Questions

What must be collected at dog boarding check-in?
Vaccination records, feeding instructions, medication schedule, emergency contact, veterinarian contact, behavioral notes, exercise instructions, and signed boarding agreement.
Should I collect the owner's vet contact at intake?
Yes. In a medical emergency, having the vet contact allows you to proceed with care if you cannot reach the owner. Collect practice name, phone, and the attending vet's name.
How do I handle a dog who arrives without vaccination records?
Do not accept without records unless you have a pre-established policy exception. Offer to hold the reservation 24 hours while the owner contacts their vet. This protects your other dogs.
What should be in the boarding agreement?
Vaccination acknowledgment, feeding and medication authorization, emergency medical care authorization with dollar limit, photo consent, cancellation policy, and liability limitations.
